Network Facilitator
Croydon
£12.56 per hour
Urgent – Interviews to be held on Friday, 14th December
Within this role your primary responsibility will be to facilitate multi-agency working with GPs, practice staff, community nurses, social workers, pharmacists, and PICs, as well as services including statutory and non-statutory organisations.
Duties will cover the following:
- Ensuring multi-agency meetings at GP practices in Croydon take place at the agreed frequency; there may be a requirement to facilitate the agreement of alternative meeting dates with the team when these need to be re-scheduled to optimize attendance;
- Collating information on new people to discuss and recording this on Excel;
- Recording and monitoring actions agreed at multi-agency meetings on Excel;
- Timely communication with the multi-agency team on actions agreed, usually via email but it could be via phone;
- Escalating non-attendance of team members at multi-agency meetings;
- Being the point of contact outside meetings; and
- Capturing data required for the Key Performance Indicators (KPIs).
The candidate needs to:
- Have accurate typing skills;
- Be able to use Excel for recording information;
- Be able to use email;
- Be confident working with health and care professionals (social workers, GPs, community nurses, pharmacists and personal independence coordinators);
- Be comfortable with handling sensitive and complex personal information;
- Have an understanding of information governance regulations; and
- Be willing to work in various locations across Croydon (in GP practices).
Ideally the candidate should be a car driver, but this is not essential as buses can be used to move between the office and the practices.
